You will need to provide the following for your visit:
1. Proof of age and name (i.e.; driver’s license, birth certificate, school ID, etc.)
2. Wear a mask
3. Be prepared for the process to take 45 minutes.

Texas law requires parental/guardian consent for the vaccination of children under the age of 18. Parental/Guardian consent may be given orally or in writing, the parent/guardian does not need to be present for the adolescent to be vaccinated but will need to be available via phone for consenting purposes.

Booster Vaccinations are available if you meet the following: you are immunocompromised (transplant, cancer, HIV, or currently taking high dose corticosteroids or other drugs that may suppress your immune system) and your last Covid 19 vaccination was at least 28 days ago, by scheduling your booster appointment you have attested that you meet the requirements.
